The PCMH health care delivery model can best be described as a patient-centered model designed for patients to receive the right care at the right time, lowering health care costs and improving the health care consumer’s overall health across the health care system. For health care providers it can mean improved collaboration, communication, and improved patient outcomes.  Post Rock Family Medicine is proud to be one of the pilot practices in Kansas working to transform our practices from good to great.

On Friday, January 27, PRFM providers and staff spent the day in Salina with members from the other seven pilot sites.  The day was designed to share information between sites from the first six months of the project.  We learned from each other’s successes and challenges alike.  All of the PRFM attendees came back to Rooks County with new ideas and more energy for the project.

PRFM staff also have a sense of humor!  The group is pictured in new t-shirts, which were designed to give one of the fearless leaders (ahem) a little grief.  Stop by to see our new attire or ask us about the exciting changes we are making on our PCMH journey.
