Million Hearts Initiative

Million Hearts™ is a national initiative to prevent 1 million heart attacks and strokes between 2011 and 2017.  The Department of Health and Human Services (HHS) launched the initiative in an attempt to combine new and existing programs to improve health and aid in Americans living longer and more productive lives.  The Centers for Disease Control and Prevention (CDC) and Centers for Medicare & Medicaid Services (CMS) are taking the lead within HHS to make a long lasting impact against cardiovascular disease.

The fight against heart disease begins with recognition.  1 of every 3 deaths in America can be attributed to cardiovascular disease.  Everyone must be aware of their risk factors and maintain a relationship with a physician who can help recognize high blood pressure (hypertension) at an early stage.

People who need treatment should be encouraged to focus on ABCSAspirin for people at risk, Blood pressure control, Cholesterol management, and Smoking cessation; all of which target major risk factors for cardiovascular disease.

Post Rock supports the initiative by working closely with CMS and comparing local data with national data.  Further, our team consistently reviews patient data with our weekly Quality Improvement initiative to determine where we can improve our care and how we can make sure that all of our patients have each of their specific needs met.  We do so in our bi-weekly staff meetings in reviewing various chronic illnesses and the care provided, showing where we can improve and how we have done over time.
